Case Management
Personal Care Consultants have been providing multi-disciplinary case management since the company's inception in March 1997.
Our multidisciplinary team of social workers, occupational therapists, registered general nurses with associate input by physiotherapists, speech and language therapists and psychologists are fully conversant with current best social and health care practice.
Our multi-disciplinary case management services cover a spectrum from assessing, outcome focused and person centered care planning and / or advocating for the individual concerned. PCC are able to co-ordinate service provision and liaise with legal, medical, statutory, private and voluntary sector agencies.

Our professional front line staff are backed up by our Practice Manager and Administrative Team based in our Chester office.
As PCC have a range of professional front line staff we are able to select the most suitable occupationally competent case manager to adopt the lead case management role.
PCC work within the domain of children and adults. Cases that we have been involved in can range from clients with complex musculo-skeletal disorders, those with psychological and emotional disturbance, clients with sensory impairment to clients with spinal cord injury or acquired brain injury.
